Before Your Visit

Welcome to the University of South Florida, College of Medicine, Division of Vascular and Endovascular Surgery.

For your convenience, we have included a New Patient History Form and a map to our Harbourside Medical Tower location, which indicates our free valet service.

We are committed to working closely with your referring provider to aid you in the referral process. After your visit, a report of the findings will be sent to your referring provider/primary care provider and other referral sources. If you have provided their names and addresses.

Please bring with you:
  • Copies of your medical records that are pertinent to your visit. These may be from your referring physician, primary care provider, hospital or emergency room records. YOU MUST HAVE actual xrays/CDs for all xrays, MRI, CT, angiograms, or any other testing pertinent to your visit.
  • A list of names, addresses, phone, and fax numbers for your referring provider(s), primary care physician, or any other provider you wish to receive a copy of your visit notes
  • Your insurance card or policy
  • The completed New Patient History Form
  • A list of all current medications and the dosage
The above information is to reach the next generation of healthcare providers in an environment that fosters excellence in education, research, and compassionate patient care. Because we are an academic medical group, our faculty providers lead patient care teams that may include medical students, residents, fellows, and other healthcare providers. The providers you may encounter on your visit include:
  • Medical students: Students training to become physicians through a four-year course of study for a medical degree at the USF College of Medicine.
  • Residents: Physicians training for a certain surgical specialty.
  • Fellows: Physicians who have typically gone beyond residency training and are continuing their studies in a subspecialty area of their field.
  • Advanced Registered Nurse Practitioners (ARNP): Registered nurses are qualified through advanced training to assume some of the duties and responsabilities formerly performed only by a physician.
As a patient, you help us educate tomorrow's healthcare providers while gaining access to the latest knowledge and advances of medicine. We appreciate your cooperation with our teaching efforts.
